- A Care Plan that recommends appropriate services and supports
Care Management
 Arrangement and coordination of services, referrals, and assistance with government programs when necessary- Continued monitoring of the older adult and intervention when circumstances change
- Relocation assistance
- Liaison and support for clients’ families who live at a distance
- Member of the National Association of Professional Geriatric Care Managers.

Home Care
 Certified Home Health Aides provide:
- personal care services
- medication supervision
- meal preparation
- assistance with errands, appointments and social activities
- assistance with household tasks
- The Home Health Aide’s care is supervised by an R.N.
- State certified health services firm
- Bonded, insured, workers compensation
- Charter member of the National Private Duty Association
Court Appointed Guardians and Conservators
- For older adults who have been deemed incompetent by the court, Senior Care Management will serve as a guardian. We also serve as court appointed conservators.
